Transitioning Services

Once you have your person-centered plan, it's time to present it to your regional center. We can assist you in navigating the steps of transitioning process from traditional services to your self-determination program. The process will include scheduling an IPP meeting with the client, supporting individuals (e.g., parents, guardians, etc.), independent facilitator, and service coordinator. This support will begin after the consumer has attended an SDP orientation and before the consumer is enrolled in SDP.
